This program provides community-based case management and outreach services to Burlington County adults who are dealing with a serious and persistent mental illness and are either homeless or at risk of becoming homeless. This program also provides targeted case management services for military veterans experiencing homelessness and mental health issues. Individuals eligible for PATH must meet HUD's (Department of Housing and Urban Development) definition of homelessness, experience chronic homelessness, be able to provide a certification of homelessness from a community provider that provides homeless services, or are at imminent risk of homelessness (within 14 days).
